--- name: eso-pediatric-obesity-rearing-patterns description: This skill guides clinicians to identify maladaptive rearing patterns related to diet, activity, and screen time in pediatric patients with overweight or obesity. It prompts evaluation of screen time, dietary habits, physical activity, and family eating cues to target education on healthy food and exercise habits. --- # Identify maladaptive rearing patterns related to diet and activity ## STEP 1 — Gather Information Collect data on daily non‑academic screen time, types and frequency of snacks and beverages (especially sugary drinks and fruit juice), fruit/vegetable intake, fast food consumption, physical activity duration and type, family meal patterns, eating triggers (boredom, stress, loneliness), sleep duration, and parental modeling of habits. ## STEP 2 — Rule In / Rule Out If screen time exceeds 2 hours/day OR maladaptive eating patterns are present (e.g., constant grazing, sugary beverage intake >1 serving/day, fruit/vegetable intake <5 servings/week, physical activity <20 min/day) OR family meals are irregular and eating occurs without hunger cues, then rule in maladaptive rearing patterns; otherwise rule out. ## STEP 3 — Classify or Stratify Stratify by number of affected domains: 1 domain (mild), 2 domains (moderate), 3 or more domains (severe) where domains are screen time, diet, activity, and family eating environment. ## STEP 4 — Decide For mild patterns, provide brief counseling and educational handouts; for moderate patterns, develop a family media plan, set specific screen limits, introduce structured physical activity, and schedule monthly follow‑up; for severe patterns, refer to a multidisciplinary obesity program for intensive lifestyle intervention and consider behavioral health support. ## Clinical Guardrails / Mimics / Pitfalls Avoid focusing solely on weight loss; do not recommend restrictive diets that may cause nutrient deficiencies; do not eliminate screen time without offering alternative activities; avoid blaming parents; recognize cultural norms that shape food and activity practices; do not use screen time as a pacifier for emotional distress without addressing underlying stressors. ## Concrete Clinical Example A 9‑year‑old girl with BMI at the 96th percentile presents for a well‑child visit. Mother reports the child watches TV or plays video games for about 4 hours daily, snacks on fruit juice and crackers while eating in front of the screen, and the family eats meals together less than three times per week. Physical activity is limited to school recess. Screening reveals maladaptive rearing patterns in screen time, diet (grazing, sugary drinks), and family eating environment. The clinician educates the family about limiting screen to 1 hour/day, offers alternatives like bike rides, suggests structured snack times with whole fruits and water, and arranges a follow‑up in 4 weeks to review progress. **Source:** Pediatric Obesity Assessment, Treatment, and Prevention: An Endocrine Society Clinical Practice Guideline, Endocrine Society, 2017, DOI:10.1210/jc.2016-2573
